entering history in computer

Hi histonetters,
Wondering if you have an old card file of patient diagnosis and have to
enter old patient history in your computer to update when a patient has
a current specimen. Our card file has information that is 15+ years old.
Our computer system has information on each patient that covers all
specimen information from about 1985 to present. Do you all still have
to add old information to that patient? When do you pick a cutoff date
or do you lookup in an old card file if the pathologist requests. It has
added about a hour each day to the transcriptions duites (lookup and
entering in history portion of the
report). Thanks for your input. My staff is so busy I am looking for
time wasters I can eliminate. Thanks,
