Jill,
When any tech makes an error it is discussed and usually does not occur 
again. If the same tech consistently makes the same errors then there is a 
greater problem. This tech would then be warned verbally. If errors still 
occur then you are forced to write them up. Unfortunately we all make 
mistakes and are human. I always use these mistakes as a mode for teaching.
