If you manage by the Fear and Loathing Method, then write up the
degenerate
culprit.  On the otherhand, one may try to design proceedures in such a
way that making mistakes is difficult.  Sort of a checks and balances
system.  I've found that cosmic fatigue induced by an atmosphere of fear
and loathing makes the mind wander and increases mistakes.
