FeedbackStat.com is a market research company
that conducts new product surveys in the field of histology. We are currently
working on a project for a health care company that wants general
information about clinical laboratories.
If you work in a clinical histology laboratory, please log
on. The survey will take approximately 10 minutes. You will receive a
$10 gift certificate for your participation.

last name (lowercase) and the last four numbers of your telephone
number. If you have logged on to Feedbackstat.com before, you can
enter the username you used in the past which may be your first and last name,
all lowercase, no spaces.

If you do not have Internet access and would like a survey faxed to

Also, if for any reason you have filled out one of our surveys in the past
and have had any problems receiving your gift certificate, please call or
