Joanne,
 We consider the date of service the date that the actual procedure was performed and the specimen was taken.  The day it is received in our lab is called the receive date.  I'm not sure if that is what other labs do, but that is our procedure.

Histonetters,
I need everyones opinion as to what a "Date of Service" is considered to be.
Is it the day tissue is received in the laboratory for processing or the day
the pathologist renders their diagnosis. Please reply to the e-mail address
