Assuming that you've checked to make sure that the tissues are completely 
covered with reagents during the entire staining process, here's the first 
things that pop into my head as possible answers:

1) tissue partially drying out on the trimming table
2) tissue not completely submerged in formalin after being trimmed in
3) tissue not completely fixed all the way through
4) tissue not completely deparaffinized
5) tissue undergoing faulty antigen retrieval step
