88312 can be charged for each different organism stain performed per
specimen.  There can be multiple charges per case if they are from different
specimens, ie A, B, and C.  If you do a gram and AFB on specimen A you can
charge 88312 x 2.  If you stain several slides on specimen A you may still
only charge 88312 one time per type of stain.  If you stain a gram and AFB
on specimen A and B you may charge 88312 x 4
