That's one of the easiest questions ever posed here.

I simply say so in the report. If it is possible to give some indication of the likely cause, that is mentioned.
So a report may go something like....
"Regret material did not survive processing - possibly due to its being mucin only."

Our pathologists have given us the task of benchmarking "lost specimens".  We are trying to find out what other labs do when something does not survive processing.  Any info. would be greatly appreciated.
