We have a check of cases before they leave the lab at allocation area. The
tech who send the slide to pathologists doubel check. To ensure that the
pathologists do their check wothout much fuss we place our labels below the
number on the slide so that the slide number and the label number can be
seen at the same time. This gives no excuse for the techs or pathologists no
noting. We also encourage our filing clerks to report any missed cases.
