The CAP website has a section that answers coding questions.  To see their
reply for specimens submitted in one bottle go to
www.cap.org/html/advocacy/capdocs/ct0799a.html.  In a nutshell, if the
specimens in one bottle are individually  identifiable (ink, suture), or
are grossly distinguishable and require a seperate diagnosis for each then
you can charge for each one.  If, for example, you receive multiple skin
tags in one bottle, and only one slide is prepared and one diagnosis
rendered, you should just charge for one.  Another example they gave was if
you receive axillary contents in one container, and the breast in another,
it is still a modified radical mastectomy, and should only be charged as
