Sheila, We have a three-ply form that the pathologists submit to us when
they order special stains.  We use that form to document who performs,
bills, the stains.  We then submit that same form back with the completed
slides to the pathologist.  The form has a table with each stain listed.
Also in the table are 8 columns.  The two main columns are categorized are
"patient" and "control".  These two columns are then divided into 4 columns
each and they are labeled "N", 1+, 2+, and 3+.  The paths fill out these
forms and then return them to the Histology Department when they are done.
This has been satisfactory as far as are inspections are concerned when it
comes to reactivity of controls.
