We use prefilled formalin containers for the smaller specimens and biopsies.
Surgery, radiology, endoscopy, and the nursing floors use the prefills and
label with the patient information.  For the larger specimens we use empty
containers which surgery fills with formalin and labels with the patient
information and a formalin/biohazard label. Surgery orders the prefills or
empty containers which are stocked by the hospital materials management
dept.  Histology provides containers to endo, radiology, the floors.
