We do 3 levels on stereotatic breast biopies but not on mastectomies or
lumpectomies.  All bx's get 3 levels(approx. every tenth section).  If they
want additional levels we have a sheet that they request them on.  We do
duplicate slides on lung and prostate bx's.  Endometrial currettings we only
do one level if it is more than one block.  If it is one block we do 3
levels.  on serial sections, the pathologist's rquest the number of slides
but only after he has looked at the original slides.  I hope this is some
