I read the 3 requests for CISH procedures to be faxed, so I transferred the emails to my email at the hospital, but it didn't pick up the full text. I will be happy to fax the procedure, can you email me at ckiely@nmh.org instead of ckiely128@aol.com which is my home. I think 1 request was from IU, and can't remember the rest. Thanks.
