we deal with this by requiring that each specimen being submitted to our lab
have the patient billing information provided, especially for medicare, this
request for billing info is right on our specimen requestion form along with
submitting dr. etc. it is the responsiblility of the dr. submitting the specimen
to us to see that the patient provides us with the billing information on this
form.  if it is not there, i go back to the dr. and tell them they have to
provide this or we cannot process the sample.  we definately get forms that are
not properly filled out, but usually after the dr. is contacted a time or two
they see that it gets done in the future.  it is so much easier to get this
upfront than to try and track down the patient after the fact.
