RE: Medicare compliance

From:"Carson, Karla" <KCarson@chw.edu>

It was during a Medicare audit that this issue came up. Our Regional
Compliance officer informed me that there would be no payment since the
attending had not written the path order in the patient chart. I put the
question back to her that keep the requisition and have no influence over
what orders are written in patient charts. They have not yet responded.

We have been doing that also, but was just told in a
recent audit
		that
wasn't good enough. They wanted to see the order
written in the
		chart.
			Haven't resolved this problem yet.

We have a "Special Studies" section
in the report
		that we
			count as
					documentation for special stains,
immunos, consults,
		etc.
			and count this as
the order. We also attach the order,
from
		pathologist or
			clinician to the
requisition which we keep for 10
years.

						Wondering how the rest of
the Pathology
		World
			handles the regulation
					that
						states, " all tests
performed in the
		laboratory must
			have a doctors
					order
						and must be documented in
the patients
		chart." Are
			you using a
					seperate
form when you get orders for
special stains
		or
			immuno, or is the
					Pathology
						report considered enough
documentation??????
