I have seen many of these forms in my travels as a locum tenens pathologist.
I would estimate that about a quarter of laboratories use them. Their
presence seems to correlate with poor quality of slides. I always check them
off as if everything was wonderful, and I'm sure the Herrn Inschpektors beam
when they read them.
If the purpose of such forms is to keep the inspectors happy, then they
should be written up in whatever form the inspectors want, with as much
dispatch as possible.
If the forms are meant to improve the quality of slides, they are a poor
substitute for constant communication between pathologist and histologist.
