I can send you a copy of a blank report with the headers that I use when
I'm writing a report for an efficacy study.  I can also send you a copy
of a spreadsheet that we use for tox studies to record the
histopathology data.  For a tox study we normally would have individual
animal data sheets that cover all the diagnosis from one animal.  Then
you would need a summary table that includes all of the data and from
there a written report that explains the findings.  If you like I could
compile some samples of this for you, but its going to take me a few
days to get it together.  You can change these forms to reflect the
parameters you are looking at if you are doing pathology for a efficacy
study.
