We use Meditech Client Server and I have put all of the tumor staging
forms into the computer system (all the patient demographic drop in so
there is not a chance of transcription error, also looks better) this
way the pathologist can fill out the forms.  The forms will go to the
electronic medical record where they will live forever.  Because the
forms go to the electronic medical record the doctor has access to the
form all the time.  Also our Tumor Registry department has access to the
forms to make any documentation needed on their end.
