We made labels like this in MS-Word and print them out whenever we need
them. You can make several different sizes, and they are totally custom. You
can use many different types of label stock too.

I also did something similar with NFPA labels in a database program. it had
the diamond in different colors and all.

Also, is there anyone who can supply these with an adhesive which allows for
easy removal?  I have checked several vendors including Lab Safety Supply
and unless I have them customized they seem to be unavailable.
