From:Lara Muffley (by way of Histonet)

I've spent so much time on this very problem.
TEC-3 from DAKO is supposedly the way to go!

These refs really helped me:
 J Cell Physiol. 2000 Mar;182(3):311-22.
The Ki-67 protein: from the known and the unknown.
Scholzen T, Gerdes J.

Immunohistochemical detection of cell growth fraction in formalin-fixed and
paraffin-embedded murine tissue.

Hainfellner JA

Good luck!
