I charge a $45.00 search fee which the time to look in files and determine if there are any records and to pull if available.  This fee is charged wether or not there are any records.  It is primarily to cover the time involved in multiple phone calls and aggravation.  I then charge $15.00 per H&E stained slide and $7.50 for each unstained slide.
