Please forgive me.  This is a little off topic, but I can not help myself.  "A military commander at an Army Hospital" that is "cost effective" conscience?  Are you sure this is correct?  I don't believe that I have ever heard of such a thing in the military.... boy, times must be changing!   ;-)
