Jeff Silverman at Southside Hospital in Bay Shore NY describes very
carefully thought out and written procedures for a variety of non-gynecologic
cytopathology specimens.
Most of the small laboratories I work in have only the sketchiest procedures
written for non-gynecologic specimens, and typically what gets done depends
on the whim of the person doing cytopreparation that day, and there is no
feedback to the physician performing the procedure. The result is that these
cytologic procedures are very nearly worthless, particularly if there is no
cytotechnologist available to prepare and screen the specimens. A lot of
these services would benefit from taking Jeff Silverman's approach.
The real question we have to ask here, though, is: how much longer will small
hospitals, particularly in rural areas, be able to maintain pathology
services at all? Based on what I'm seeing out there, I'm not optimistic.
