use your first and last name (no space between the names) as the user name
and then NSH123 as the password

I tried to participate in the survey as requested, but when I accessed the 
www.feedbackstat.com website, it mentioned an NSH survey, instructed me to 
"Log in here", but then asked for a user name and password.  I couldn't get 
any further.  Am I missing something?
