Amos Brooks instructs us:

Most importantly, find who grossed the case [that didn't process right] and
KICK THEM SOLIDLY IN THE KNEE. ;-)

Amos Brooks has hit on it. It's the responsibility of the pathologist, or
whoever the pathologist supervises who grosses, to prepare the tissue so it
will infiltrate right. And it's the responsibility of the pathologist and the
histotechnologist to get together and devise a grossing and processing
routine that will assure adequate infiltration.

Or as my high school's old football cheer went:

Rickity rackity ree,
Kick 'em in the knee!
Rickity rackity rass,
Kick 'em in the other knee!
