RE: [Histonet] HIPPA and slides[Scanned]

From:"Stapf, Ross"

I seem to remember either this being discussed on Histonet before or
reading an article about this. Either way, my memory is that the
discussion about HIPPA and patient names on slides and blocks concluded
that HIPPA requires reasonable care be taken not to release information.
The slides and blocks themselves aren't information until they are
examined by a qualified person. Removing the name is not necessary
prior to disposal. Having the name as well as surgical # on the
block/slide is beneficial for proper identification.

We always, in the UK, I think put both on; it's part of our IQC. I
suppose the choice is to be sued for mixing up the slides or be sued for
releasing Patient's names; which is cheaper?

We are presently putting both on the label. It does seem against Hippa,
but the slides shouldn't end up in too public a place. AND- there's
allot of John Smiths out there, no way of telling which John Smith
you've got. Having both name and surgical number on the label has
diverted enough errors to know that this is definitely worth while.

Does anyone know if patient names can go on slides in addition to

numbers? Doing so seems like it would go against HIPPA law.
