Two poplular formulations to pick from:

Kristensens 

8N formic acid (1N formic acid is equivalent to 8M)  500 mls
1N sodium formate (6.8%)        500 ml


Evans and Krajian 

Sodium citrate 10 gm
90% (stock, concentrated) formic acid 25 ml
Distilled water 75 ml

These formulations are available from various vendors also.  Saves time
having to make up, more importantly - not having to store concentrated
formic acid
