I need to get as many examples of Autopsy Authorization forms as I can get.
You can either fax them directly to Dr. David Myerson c/o Fred Hutchinson
Cancer Research Center, 1 206 667-6129 or you can send an attached copy by
email to him at <dmyerson@fhcrc.org.> These forms will be used in
confidence and will not be distributed outside of our group. We are using
these forms to see what authorization is typically given regarding the
sharing of paraffin blocks for subsequent research by outside institutions.
We do not intend to request your blocks. I appreciate the help.
