IHC's and ASR's questionnaire Please help!

Hi everyone in Histoland,
I feel the need to stir up some discussion regarding ASR's and IHC's. :-)
We have just finished a meeting of the minds here at Baylor and we are
curious as to what are other institutions doing about ASR's.  Here are my
questions, if you would like to respond privately, no problem, I will
compile a list of the responses, I receive and present them to Histonet in a
condensed form.

1.	Are you presently placing the disclaimer on the Final Surgical
Reports that is required by FDA regulations?
2.	Are you adding any additional comments?  If so, what are you adding
to the disclaimer?
3.	How are you handling the disclaimer, when the tests are either sent
out of the hospital or if IHC's are referred in to your lab by another
institution?  In other words, if you are using an ASR are you notifying them
that an ASR was used?  How are they notifying you?
4.	Has anyone noticed if reimbursement was affected?
5.	Did you have any problems from clinicians who received the reports
with the disclaimer on it?
6.	Are you doing validation studies on ASR's?
7.	Anyone had this asked by CAP, CLIA'88 inspectors or any other
accrediting agency?

I would really appreciate your help with this and I would like to share this
with everyone else, who may be working on this as well.  I will share the
results also with NSH through the Legislative Committee.
