Thanks to all who responded to my call for help.  It was greatly
appreciated.  We are working up a complete policy for these situations.
The input is very valuable.
    Thankfully we didn't have to use a form, the guy took one look at
his finger in the bag and looked rather pale and decided he didn't want
to take it. ;)  I guess it's a little different when you don't see that
kind of thing everyday, and certainly not your own.  I was very relieved
that he made that decision.
