I include a pre-printed quality control sheet with each IHC stain I hand
out.  I fill in case & patient information and list all the IHC's performed
in a column.  The pathologist marks if it is a problem or satisfactory and
includes any pertinent comments for each.  All quality control sheets are
then returned to me so I can address any problems and I file them for
posterity.  This works very well and meets the CAP requirement.
