It is a CAP requirement that clinical info be included on the requisition.
We have an ongoing issue with this and try to
make corrections through a QI process. We do not have the surgeon sign the
requisition. However, if a clinician orders a test such as ERPR or Her2, we
do require a signature for the written order.

I have some questions for those of you who work in a hospital and

specimens/requisitions from surgery. We are re-doing the hospital

trying to determine what needs to stay on the form.
My questions are: Is there a place on the req for the surgeon to
sign, does
the surgeon sign the req, do you know of any regulatory requirements
to have
the surgeon sign the req?
