CPT 88311 (decalcification)

To charge a professional component for decalcification (88311).

Don't know what my various clients do - I'm rarely privy to such information. 

Could it possibly depend on whether the pathologist or the histotechnologist 
does the decalcification? - I very often do my own, but not always. I never 
thought of documenting this detail of the procedure in the protocol, though, 
and this documentation would be essential if one were to use this point of 
procedure to decide on charging the professional component. I guess 
"decalcification performed by Dr. Richmond" would be adequate.
